Background: Hospitalization increases the risk of thromboembolism (TE) associated with COVID-19; large geographic representative estimates are needed to better understand the scope of the problem.

Aims: To assess the rates of venous and arterial TE events (VTE, ATE) during hospitalization and up to 60 days post discharge in adult patients with acute COVID-19.

Methods: We conducted an IRB-approved retrospective hospitalized COVID-19 cohort from 2020-2023. We included adults (> 18 years) hospitalized for >1 day and with laboratory confirmed COVID-19. Data were collected by manual abstraction (7 sites) and electronic (2 sites). Outcomes included rates of VTE and ATE. Data are presented as proportions, with binomial 95% confidence intervals (CI) during index hospitalization and 60 days post discharge.

Results: The study cohort comprised 49,440 patients from nine US medical centers. (Figure 1) Participating hospitals included all four census regions of the United States: North-East (Beth Israel Deaconess Medical Center, MA, n=1021, Rochester Regional Hospital, NY, n=99), Mass General Brigham, MA, n=20,106); South (Johns Hopkins All Children’s Hospital, FL, n=27, BayCare Health System, FL, n=3754);MidWest (Cleveland Clinic Foundation, OH, n=21671, Henry Ford Health, MI, n=968,University of Illinois Chicago, IL, n=1189); and West (University of California, CA, n=605).

The median age was 62.6 years (IQR 50.5-77.6); 50% were female and 68% were White. Of these, 2,627 patients developed a VTE during index hospitalization (5.3%, 95% CI 5.1-5.5) and 883 (1.8%, 95% CI, 1.6-1.9) ATE. Of 39,442 patients for whom post-discharge data were available, 1,131 (2.9%, 95% CI 2.7-3.0) developed VTE and 534 (1.4%, 95% CI 1.2-1.5) ATE up to 60 days after discharge.

Conclusion(s): In this national multicenter cohort study, a high incidence of VTE and ATE was noted during hospitalization for COVID-19 and persisted for up to 60 days post-discharge. Accurate estimates on thrombotic rates and trends can help plan targeted thromboprophylaxis interventions.
