Background: Accurate and precise factor VIII (FVIII) and factor IX (FIX) measures are essential for the diagnosis and the management of haemophilia A and B. A reliable Factor VIII (FVIII) and Factor IX (FIX) measure over time is therefore necessary for patient’s safety and optimization of resources. This became particularly important after the lauch of the new EHL concentrates. To verify the quality of the measures the Italian Association of Haemophilia Centres (AICE) started in 2018 the External Quality Assessment Programme (EQAP). The AICE EQAP was agreed with ECAT Fundation (Veershoten, NL) and developed with particular attention to the haemophilia center laboratories and offered for free by AICE. ECAT sent 4 sample kits per year to all registered laboratories. VWD diagnostics was added from 2019. Type and periodicity of the tests are detailed in table 1.

Aims: To investigate the participation by laboratories supporting the clinical centres affiliated to the to the AICE EQAP.

Methods: We analyzed the participation data provided to us by ECAT and counted the participation in each individual test by each laboratory on a MS EXCEL spreadsheet.

Results: 44 laboratories registered for the program but only 38 participated for the full five years. Median participation was 90 % (FVIII and FIX OSA, inhibitors to FVIII and FIX); FVIII chromogenic assay (CHA) increased from 30 % to 60 % and FIX CHA from 10 % to 30 %. VWF:CBA is still used only in 40 % of laboratories.

Conclusion(s): We recorded a great participation of the registered laboratories. During the 4 years of operation we have seen a significant increase in the use of chromogenic tests, which translates into a better possibility of monitoring the treatment with new EHL concentrates of FVIII and FIX and ultimately leads to a higher quality of care and better use od resources.
