PB0804 - The effect of clinically used antiplatelets drugs and flavonoid metabolite 4-methylcatechol in diabetes mellitus type I patients and generally healthy volunteers.

Background: There is no doubt that high glucose levels in diabetes mellitus is one of the main risk factors for cardiovascular diseases and higher platelet aggregability is a known factor in those patients 1,2. More to that, platelets from diabetic patients can be resistant to clinically used antiplatelet drugs rendering the therapy more challenging 3,4.

Aims: Flavonoids have been suggested to have positive effect on cardiovascular system. In this study were studied a flavonoid metabolite, 4-methylcathecol which is interesting for its strong inhibitory effect on platelets 5-7.

Methods: Materials and methods: 50 adult patients with diabetes mellitus type I (DM1) were enrolled and a control group of 50 generally healthy age-matched volunteers was included for comparison6. Platelet aggregation was tested with 7 different agonists, and 3 clinically used antiplatelet drugs (acetylsalicylic acid /ASA/, ticagrelor and vorapaxar) and mentioned experimental compound 4-methylcatechol were assessed ex vivo by impedance aggregometer.

Results:

Results: Higher glucose levels correlated, unexpectedly, with lower aggregation responses to 4 of the 7 agonists used and similar result was observed when DM1 patients were compared to controls in 3 agonists. Additional analysis confirmed that lower aggregation responses were observed to 2 aggregation triggers in plasma samples with glycemia over 7 mM. In contrast to approved drugs, the effect of 4-MC was higher in DM1 patients than in healthy controls.

Conclusion(s): Higher glycaemia was associated with lower platelet reactivity, the resistance to clinically used drugs was observed at the same time. In addition, 4-MC seems to be a promising antiplatelet with no ex vivo resistance in diabetic patients.
