Background: Venous thromboembolism (VTE); including deep vein thrombosis (DVT and pulmonary embolism (PE) pose a major health burden, affecting approximately 10 million people worldwide with significant mortality and morbidity. PE is the third leading cause of cardiovascular death after myocardial infraction and stroke, causing more than 3 million deaths/year globally. The pathophysiology of PE is poly component, involving blood cellular and humoral elements along with endothelium.

Aims: This study was designed to profile biomarkers of coagulation parameters and hemostatic aberration in patients with clinically confirmed PE.

Methods: A prospective cohort of PE under an IRB approved project from Loyola University Chicago, was compared to normal healthy controls (25 male and 25 female), further stratified on the ESC PE severity. Citrated blood samples from PE patients (n500) were collected within 72 hours of the diagnosis. Plasma samples were retrospectively analyzed for D-dimer, PAI-1a, tPA, TAFIa, vWF, CRP, IL6, TNFα, P & E-selectin, coagulation factor VII, IX, X, XIIIa, F1+2 and TAT by using commercially available Sandwich ELISA kits. Applicable statistical analysis was performed by using GraphPad Prism Software.

Results: The PE patient cohort included 51% male and 49% female, with the median age 64-years, weight 87.6-kg and BMI 29.8-kg/m2. PE patients showed upregulation of the biomarkers of coagulation parameters and hemostatic aberration compared to normal controls except for FVII and FXIIIa (table 1). In correlation analysis, varying levels of positive and negative correlation were noted between various biomarkers (figure 1). Most of the biomarkers were significantly elevated in the high-risk PE patients compared to low-, and intermediate-risk PE, with exception of FVII and FX.

Conclusion(s): The profiling of these biomarkers confirmed the hypothesis that pathophysiology of PE is multicomponent and involves thrombo-inflammatory pathways and fibrinolytic deficit. Furthermore, this multiparametric profiling may help in the risk stratification and development of new drugs for the management of PE.
