PB1243 - SPANISH REAL-LIFE EXPERIENCE WITH rIX-FP IN SURGERIES AND PROPHYLAXIS

Background: Prophylaxis regimen with extended half-life (EHL) factors, in patients with hemophilia B (HB), allows to reduce treatment associated burden. In addition, in the context of surgeries, clinical trials showed effective and safety haemostatic control with rEHL-FIX treatments.

Aims: To describe real-life experience with rIX-FP at Hospital Universitario Cruces and Hospital Txagorritxu.

Methods: This study includes the real-life experience of two Spanish centers, with 13 patients treated with rIX-FP on-demand or prophylaxis, of which 2 of them underwent to a major or minor surgery (in total 4 procedures).
The treatment switch to those who are on prophylaxis was guided by individualized pharmacokinetic study (WAPPS Hemo platform).

Results: All patients were on treatment with rFIX-SHL (short half-life) or plasma derivated factor and the average follow-up was 33 months.
There were 9 patients with severe HB and 4 with moderate HB, of which nine were previously treated on a prophylactic regimen and four on-demand regimen. A description of the patients is shown in Table 1.
After 33 months of follow-up, improvement in the quality of life (QoL) were significantly increased and hospital burden has considerably decreased as patients have improved their joint pain and their arthropathies with reduced number of infusions. The ABR (annualized bleeding rate) was 0, except for one patient who presented a psoas hematoma secondary to intense physical exercise.
In the context of surgeries, two patients underwent for four minor or major surgeries (Table 2) with excellent haemostatic response and neither patient developed inhibitor.

Conclusion(s): The switch to rIX-FP significantly improved the QoL of patients, reducing the number of infusions and maintaining an adequate safety profile. Moreover, patients who underwent surgery showed an excellent haemostatic response. In conclusion, rIX-FP treatment demonstrates effective prevention and control of bleeds in prophylaxis, on-demand, and surgery.
