Background: Normally, pregnancy is characterized by the development of a hypercoagulant condition, this becomes especially noticeable in the 3rd trimester. Many hemostatic factors have been well studied during pregnancy, but at the moment the literature data on the physiology of the functioning of the ADAMTS-13/vWF axis are very contradictory.

Aims: to study the features of the functioning of the ADAMTS13/VWF axis in the mother during the physiological course of pregnancy in the I,II and III trimesters.

Methods: We prospectively examined 63 women with physiologically occurring pregnancies in the I, II and III trimesters.

Results: The average age of all pregnant women surveyed was approximately 28 years. The median ADAMTS-13 antigen content in physiological pregnancy was 0.901 [0.890; 0.97] U/ml in the first trimester, 0.855 [0.726; 0.92] U/ml in the second trimester, 0.787 [0.72; 0.89] U/ml in the third trimester and 0.909 [0.886; 1.00] U/ml in healthy non-pregnant women. Our study demonstrated a statistically significant decrease in ADAMTS-13:Ag starting from the second trimester of gestation (p < 0.01). The vWF median level:Ag was especially elevated in the third trimester of gestation at 1,869 [1,74;1,91] Units/ml. vWF:Ag increased by 42% in the second trimester of physiological pregnancy, and by 51% in the third trimester compared with the control group.

Conclusion(s): these data are very important, because if a woman enters pregnancy with the level of ADAMTS-13 antigen already at the lower limit of the norm, then in the second trimester of pregnancy this level will be lower than the control values and may lead to the development of pregnancy complications, such as, for example, transient microangiopathy or preeclampsia.
