Background: Different studies show that mild and moderate Haemophilia A and B (NS-HA/HB) patients suffer joint bleeds and some develop haemophilic arthropathy that can lead to surgery. For this, different guidelines recommend extending prophylaxis to these patients with bleeding phenotype and/or established arthropathy

Aims: Evaluate during 24 months, the impact of prophylaxis in NS haemophiliac patients (12 months before) and the switch to extended half-life Factor Concentrate (EHL-CF) (12 months after) in hospitals from Spanish HemoNorte Working Group

Methods: Data collection from electronic medical records

Results: We included 36 NS patients (25 HA and 11 HB) without history of inhibitors. 76 % HA patients are moderate and 46,1%, suffer hemophilic arthropathy, with average age of 34.3 years (y). 88% of patients were on previous prophylaxis, mostly with Standard Hal life FVIII, three times a week (50 %). Of patients performing switch, they were on prophylaxis, mainly with Efmoroctocog Alfa® (32%), significantly reducing infusion frequency and weekly factor consumption. This led to significant reduction in total, spontaneous, and joint bleeding rates (ABR) (See Table 1-2). Adherence improved (> 90%) in 5 patients and remained high (> 90%) in the others.
In NS-HB patients (average age 39 y, 8 moderate and 2 mild), we found a similar impact of switch to EHL-FIX (mainly Eftrenonacog Alpha®, 50%), with reduction in infusion frequency and weekly factor consumption, leading to significant decrease in total, spontaneous, and joint ABR (Table 1-2). > 90% Adherence in 100% of patients post-switch.
The main reasons for switching in both groups were clinical reasons (68.9%) and/or physician/patient preference (63.04%).

Conclusion(s): In our opinion prophyaxis is an interesting option for NS haemophiliac patients, specially when they are bleeders and the use of EHL products is effective to reduce bleeding rates (mostly spontaneous bleeds, specially moderate HA and NS-HB), infusion frequency, improving adherence and quality of life
