PB0706 - Real-life multicenter experience with the use of Avatrombopag

Background: Avatrombopag is a thrombopoietin receptor agonist (TPO-RA) indicated for severe thrombocytopenia in adult patients with chronic liver disease (CLD) scheduled for an invasive procedure and for chronic primary immune thrombocytopenia (ITP) in adult patients who do not respond to other treatments.

Aims: The main objective is to report the experience of Avatrombopag in patients from three tertiary reference centers.

Methods: A total of 31 patients who started Avatrombopag from August 2022 to December 2023 were analized: 27 patients with chronic ITP, 3 patients with CLD before surgery, and 1 patient with MDS.

Results: For patients who started Avatrombopag for chronic ITP, 58% were women, and 42% were men, with a median age of 60.6 years (19-90). The median number of previous lines of treatment for patients was 2.29 (1-8). All patients diagnosed with ITP had previously received corticosteroids, and 42% had received another TPO-RA previously. The initial dose was 20mg/24h, with 64.51% of patients requiring dose adjustment. The 70.96% of the patients achieved complete response, 12.90% partial response and 16.12% showed no response.
For patients who started Avatrombopag for CLD in a peri-surgical context, all were men. The initial dose in two cases was 60mg/24h for 5 days. In two cases, a partial response was obtained, and in one case, a complete response.
The patient with MDS who initiated treatment with Avatrombopag was in the first line to reduce platelet transfusion support, although no response was achieved.
Overall tolerance was good, with only one patient experiencing self-limiting headache, alopecia, and slight asthenia. No thrombotic events were reported, and just four patients with minimal hemorraghic events.

Conclusion(s): Oral administration and the absence of interactions with food have allowed good tolerance in our patients, with a faster onset of action than other TPO-RAs. It has gained widespread acceptance among patients, ensuring good adherence to treatment.
