Background: Antiphospholipid syndrome (APS) is an acquired disorder presenting with manifestations like venous or arterial thrombosis and/or fetal loss. Laboratory abnormalities include elevated levels of antibodies - anticardiolipin [aCL], beta-2 glycoprotein I and/or Lupus anticoagulant. It may be primary or secondary .

Aims: To study the clinical manifestations in APS
To determine the frequency of positivity for LA. ACL and B2 GP I
To study the association of clinical presentation with lab findings
To analyse the frequency of presentation of APS among clinical specialities

Methods: The APS cases were analysed for clinical presentation and laboratory findings and classified as triple, dual or single test positivity for APLA. The association of venous thrombosis, arterial thrombosis and fetal loss with triple and dual positivity was evaluated. The statistical significance of various parameters was analysed by Chi square test.

Results: Of the 2057 cases referred with clinical suspicion of APS for APLA testing over 17 months, 575 cases (27.9% ) were confirmed as APS based on lab detection of LA/ ACL/B2GP1. The prevalence of LA, ACL and B 2 GPI in age groups, gender and the clinical specialities was analysed. The association of venous or arterial thrombosis, fetal loss and SLE was correlated with LA, ACL and B2GPI positivity. The results are summarised in Tables 1, 2a and 2b.

Conclusion(s): APS commonly affects young adults with a female predominance. Secondary APS, due to SLE was most common . In APS patients, B2GP I and ACL positivity was more common than LA positivity. Venous thrombosis, arterial thrombosis and fetal loss were the most clinically significant presentations. Triple postivity was more common in SLE. Fetal loss is more commonly associated with LA and B2 GP I and venous thrombosis with B2 GP I positivity. Venous thrombosis with APLA positivity was common among hematology and neurology specialities
