PB0254 - Experience using damoctocog alfa pegol during surgery at the Bonn Center

Background: Damoctocog alfa pegol (BAY 94-9027, Jivi®) is an extended half-life Factor VIII (FVIII) treatment indicated for use in previously treated patients aged ≥12 years with hemophilia A, including for the perioperative management of bleeding.

Aims: We report real-world data on the use of damoctocog alfa pegol during major and minor surgeries.

Methods: This is a retrospective analysis of data from people with hemophilia A treated at the University Clinic Bonn between September 2019 and August 2023 who received perioperative damoctocog alfa pegol.

Results: Overall, 26 surgeries were performed on 17 patients, all covered by bolus injections. Three surgeries were excluded as the three patients had also been treated with other FVIII products, and one surgery was excluded as the patient was treated by another centre after discharge. Of the remaining 13 patients, four had severe haemophilia, six moderate and three mild; seven received prophylaxis and six on-demand therapy. One patient was aged 15 years, all others were adults. In all but two surgeries (both patients had high FVIII on day before surgery), the preoperative dose was divided according to standard local procedure. The first dose (PREOP-1) in the late afternoon or evening before surgery and the second (PREOP-2) 15–30 minutes prior to surgery. Median PREOP-1 for major (n=4) and minor surgeries (n=18) were 38.5 IU/kg (range: 21–42) and 37 IU/kg (range: 14–55), respectively. Median PREOP-2 for major and minor surgeries were 54.5 IU/kg (range: 35–56) and 48.5 IU/kg (range: 27–58). Median total consumption for major and minor surgeries up to Day 14 was 699.5 IU/kg (529–819) and 315 IU/kg (218–438). Intraoperative and postoperative haemostatic responses were rated as ‘excellent’ for all surgeries.

Conclusion(s): In summary, in 22 surgeries, in a real-world setting, damoctocog alfa pegol was observed to provide excellent haemostatic coverage.
