Background: Given the growing life expectancy in people with haemophilia (PWH), the prevalence of cardiovascular disease (CVD) in this elderly population has also increased.

Aims: This study aimed to describe CVD rates in PWH and to identify any potential factors that may impact CVD risk.

Methods: We conducted a retrospective analysis of PWH A or B ≥40 years; assessing data on CVD risk factors, CVD events and venous thromboembolism (VTE). Univariate tests and multivariate regression models were applied.

Results: A total of 100 patients were included, 85% with haemophilia A and 15% with B. The mean age was 56.06±11.21 years. The mortality rate was 1% and not related to bleeding or CVD. The patient characteristics are shown in Table 1, and divided according to the presence of CVD risk factors, events or VTE. CVD risk factors were common with a prevalence of 67%, especially with hypertension in 44%. Older age (Figure 1), kidney disease and human immunodeficiency virus (HIV) infection associated with higher CVD risk factor prevalence. There were 11% of CVD events, including 5% of atrial fibrillation. Kidney disease was the only factor associated with higher risk of CVD events. There were just 2% of VTE, not associated with non-replacement therapies or surgery. Haemophilia severity and presence of inhibitors showed no association with either of the previous variables. All the results were confirmed by a logistic regression model. Only 7% of the patients were currently under antithrombotic treatment.

Conclusion(s): The prevalence of CVD risk factors, events and VTE in our PWH cohort was similar to previous reports. Our study highlights an increased rate of hypertension among PWH when compared to the general population; and identifies age, kidney disease and HIV as significant risk factors. Notably, severe haemophilia did not correlate with a reduced CVD risk, challenging the conventional view of natural anticoagulation in PWH.
