Background: Although low levels of high-density lipoprotein cholesterol (HDL-C) are not traditionally recognized as a risk factor for venous thromboembolism (VTE), recent evidence is pointing towards plausible connections between HDL function, coagulation cascade, and VTE risks. The functional attributes of HDL are linked to its lipid composition; nevertheless, the impact of lipid composition on the anti-thrombotic capacity of HDL remains uncertain.

Aims: We aimed to examine the association between HDL lipid composition and coagulation parameters in a large middle-aged Dutch population.

Methods: Using baseline measurements of 6125 participants (age, mean [SD]: 56 [5.9] years; 47.4% men) from the Netherlands Epidemiology of Obesity (NEO) study, the associations between 34 HDL parameters categorizing very large HDL, large HDL, medium HDL, small HDL, as well as apolipoprotein A1, quantified using high-throughput 1H-nuclear magnetic resistance metabolomics platform and coagulation parameters (factor [F]VIII/IX/XI; thrombin generation parameters; and fibrinogen) were investigated using linear regression analyses, adjusting for age, sex, BMI, total body fat, menopause, alcohol consumption, smoking, medication, diet index, energy intake, physical activity, c-reactive protein and GlycA.

Results: The preliminary findings revealed a particle size-dependent relationship between HDL lipid composition and coagulation parameters. Particularly, with one SD increase in the concentration of lipid composition within very large HDL, we observed a decrease in FIX activity, endogenous thrombin potential (ETP), and thrombin peak height (median β [interquartile range], FIX: -5.1 [-5.2, -5.0]; ETP: -27.2 [-31.6, -24.9]; peak: -2.7 [-3.2, -2.6]). In contrast, with one SD increase in the concentration of lipid composition within medium and small HDL, we observed an increase in thrombin peak height, FVIII, FIX, and FXI.

Conclusion(s): Our study provides initial evidence for a size-dependent relationship between HDL lipid composition and coagulation parameters. To strengthen the reliability of these results, additional validations in other independent cohorts and in vivo studies are warranted.
