PB1214 - Benefits from the use of technology in the treatment process of patients with acquired haemophilia A and haemophilia A with inhibitor – infusion pump practise, polish pilot observational study

Background: Acquired haemophilia A (AHA) or haemophilia A with inhibitor (IHA)is an ultra-rare autoimmune bleeding disorder treated with bypassing agents and immunosuppression. One of the bypassing treatment methods for HA is recombinant factor VIIa. Current practice involves regular bolus administration of recombinant factor VIIa. The half time of recombinant factor VIIa is short (2.8 h in vivo) and thus bolus administration is very frequent (every 4 hours) and time-consuming for the personnel. Infusion pump is a promising alternative, which could reduce the burden on nursing staff as well as patients while maintaining haemostatic efficacy.

Aims: Comparison of the efficacy, practicality and personnel preference between infusion pump and bolus administration of recombinant factor VIIa in patients with haemophilia A.

Methods: Three patients with haemophilia A received recombinant factor VIIa infusion pump treatment. Their haemostasis and response to treatment was strictly monitored. Nursing personnel (n=20) was surveyed on their preference and opinion on infusion pump compared to bolus administration of recombinant factor VIIa.

Results: The maintenance of haemostasis was satisfactory in the group of patients who were administered recombinant factor VIIa in infusion pump and no dosage was missed or delayed. The majority of surveyed personnel (75%) evaluated infusion pump as more comfortable and easier administration method than 4h interval boluses.

Conclusion(s): Recombinant factor VIIa in infusion pump is more effective in maintaining haemostasis and is a favourable administration method among nursing personnel. Patients and personnel will benefit from infusion pump administration of recombinant factor VIIa for haemophilia A.
