Background: Vascular disease is the leading cause of death in the United States and advances in the care of hemophilia have led to an aging population that is now developing age-related vascular complications. While recent data suggests that persons with hemophilia (PwH) have a reduced incidence of cardiovascular disease, the burden of cardiovascular complications remains substantial and costly as the treatment often requires combinations of antithrombotic therapy and factor replacement.

Aims: Describe rates of hypertension and dyslipidemia in a multi-institutional cohort

Methods: Six hemophilia treatment centers will contribute up to 200 subjects ages 18-89 with FVIII or FIX deficiency to a deidentified REDCap Cloud database. Subjects are randomly selected from four age strata to ensure adequate representation of all age groups. Five years of retrospective data will be uploaded to the database for each subject. Uploaded data include demographics, smoking status, problem list diagnoses, medications, vital signs, and laboratory values. Each site’s institutional review board approved a waiver of consent for this study. Unrestricted funding for this study was generously provided by Bayer Healthcare Pharmaceuticals.

Results: Two hundred subjects have been enrolled to date with an additional 600 anticipated in the next 4 months. Twenty percent of the subjects were born female. Ages range from 20 to 89 with 27% of subjects being over the age of 60. Sixty-five percent of the cohort has FVIII deficiency and 2 subjects have active inhibitors. Twenty-four percent of subjects actively smoke and an additional 19% are former smokers. Twenty percent of the cohort were diagnosed with hypertension and 15% were diagnosed with dyslipidemia. Five percent of the cohort has established coronary artery disease.

Conclusion(s): This study provides a cross-sectional snapshot of cardiovascular risk factors among PwH at multiple institutions and will serve as a baseline for future quality improvement efforts.
