Background: Cancer patients have a higher risk of thrombosis than the normal population, as well as an increased risk of mortality and morbidity.

Aims: To determine the incidence rate and risk factors associated with cancer-associated thrombosis (CAT) in patients with solid cancers in Malaysia.

Methods: A prospective, observational study was conducted at HCTM and HKL in patients aged ≥18 years with solid cancers and followed up for six months. Data on patient demographics, cancer parameters and thrombotic events were collected and analyzed using SPSS Version 28.0 software after informed consent. This study was approved by the Medical Research and Ethics Committee of the Malaysian Ministry of Health and UKM.

Results: A total of 250 patients with solid cancers were recruited. Twenty-seven patients (10.8%) developed CAT with three arterial thrombosis. The mean age and BMI of the cancer patients with CAT were 53.5 years and 26.5 kg/m2, respectively. About two-thirds had an ECOG performance status (PS) 0-1 and majority were newly diagnosed cancers (96.3%). The most common malignancies associated with CAT were pancreatic, lung, gynaecological tumours and germ cell tumours, most of which were stage 4 (p=0.01). Risk factors for CAT in patients with solid organ cancers were ECOG PS 3 (adjusted odds ratio (aOR): 12.184; 95% CI: 1.885,78.752), Khorana score ≥3 (aOR: 6.892; 95% CI: 1.51,31.47), multiagent chemotherapy (aOR: 4.363; 95% CI:1.097,5.545) and patients who received platinum-based chemotherapy (aOR: 6.181; 95% CI: 2.175,17.568), as shown in table 1.

Conclusion(s): In our study, the six-month incidence of CAT was 10.8%. Risk factors for CAT included ECOG PS 3, a high Khorana score, platinum-based, and multiagent chemotherapy. CAT risk assessment should be performed in all cancer patients to identify high-risk patients who may benefit from primary thromboprophylaxis.
