Background: Fatigue is a common symptom among patient with primary immune thrombocytopenia (ITP).

Aims: To provide real-world insights into fatigue in a well-characterized cohort of adult ITP patients.

Methods: Fatigue levels were evaluated in adult primary ITP patients from the Vienna ITP Biobank (EC 1843/2016) using the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F) and compared to established norm groups (NG). FACIT-F includes 13 items a 5-point scale (0:”Not at all” to 4:“Very much”) measuring fatigue in the past week (total score range: 0-52). Higher scores represent less fatigue.

Results: FACIT-F was assessed in 73 adult ITP patients (newly diagnosed 27 (37%), persistent/chronic: 46 (63.0%)). Patient characteristics and FACIT-F data are shown in Table 1. ITP patients showed significantly lower mean FACIT- F scores than NG (mean±SD? 39.0 ± 11.8 vs. 43.5 ± 8.3, p = 0.031). Newly diagnosed ITP patients tended to have more severe fatigue than chronic patients (37.9 ± 11.6 vs. 39.6 ± 12.0, p=0.570). The mean fatigue scores tented to be lower for females than males (38.6 ± 12.7 vs. 41.6 ± 10.3 p=0.72). Patients undergoing treatment had lower FACIT-F scores (35.1±13.11 vs. 40.8 ± 10.7 p=0.05).
FACIT-F score showed a moderate negative correlation with SMOG Index (r = -0.322, p = 0.005), weak positive correlation with platelet counts (r = 0.240, p = 0.042), vitamin D levels (r = 0.301, p = 0.018) and moderate with ferritin levels (r = 0.375 p = 0.042). No correlations were found with sex, age, disease duration and hemoglobin levels. Multivariable regression analysis identified ferritin (p= 0.034) as the only significant predictor of fatigue (Table 2).

Conclusion(s): In our cohort, ITP patients had more severe fatigue than NG, which was more deteriorated in patients with newly diagnosed than chronic patients. Levels of ferritin predict fatigue, suggesting intervention possibilities.
