Background: Cardiovascular diseases (CVDs), mainly ischemic heart disease and stroke, are worldwide a leading cause of mortality and account for approximately 18 million deaths each year. Recognizing risk factors constitutes an important issue for acting on modifiable behaviors and reducing the personal risk of CVDs, even in healthy subjects.

Aims: To evaluate risk factors of CVDs among lifestyle and socio-demographic factors in a large prospective cohort of healthy subjects from HYPERCAN study and their role in the prediction of CVDs diagnosis.

Methods: From November 2012 to June 2022, 10,291 blood donors were enrolled, of which 8,622 completed the questionnaire. Follow-up information on study outcome was available for 7,916 subjects, and lifestyle behaviors have been evaluated and correlated with CVDs.

Results: During a median follow-up of 87 months (6-120), 140/7,916 subjects suffered of CVDs (124 M/16 F median age 53), specifically 53 myocardial infarction, 28 angina, 23 coronary heart disease, 26 ischemic or hemorrhagic stroke, 3 aortic dissection or rupture, and 7 peripheral arterial disease. Cox regression identified age>50 years (HR: 3.67), male gender (HR: 3.77), smoking habits (current vs never smoker HR: 1.96), district of residence (city vs mountains/lakes HR: 1.53), and hypercholesterolemia (HR: 1.47) as independent risk factors of CVD. Of note, no association was found with high blood pressure, lack of physical activity, and CVD family history. The 5 independent risk factors were used to generate a CVC risk model able to effectively identify those subjects at high CVC risk (C-index=0.71).

Conclusion(s): This analysis has identified, in a cohort of low-risk subjects, CVDs risk factors which can differ from those of high-risk subjects according to European Society of Cardiology guidelines. This is notable, since a key strategy that may prevent many cardiovascular events lies in the recognition and therapeutic modification of “bad” lifestyles also in a healthy population.
