PB0792 - The effect of aceclofenac and etoricoxib on platelet aggregation activity in bone trauma

Background: Platelets are multifunctional cells that provide hemostasis and primary repair clot formation during injuries and surgical operations. Nonsteroidal anti-inflammatory drugs, in particular aceclofenac and etoricoxib, affect platelet function.

Aims: Study the effect of aceclofenac and etoricoxib on platelets aggregation function in bone injury.

Methods: 60 Wistar rats aged 6 months were divided into 3 groups (n=15 each), gavaged with: ACF group –1ml aceclofenac suspension (1,5mg), ETC –1ml etoricoxib suspension (1,5mg), control group – 1ml normal saline solution. Intragastric administration was carried out 7 days before surgery and after operation until the end of experiment. Bone trauma - a through round hole in femur with 1,5 mm diameter, was modeled. Rats were removed from the experiment after 7 days of intragastric administration, without surgery (0 days), 1 and 7 days after operation (5 animals each period). Platelet count was determined using hematology analyzer «MEDONIC M 20», ADP-induced aggregation parameters - on “Biola” optical aggregometer. For statistical processing, Kruskal-Wallis test and multiple analysis were performed. All experiments were conducted in accordance with Helsinki Declaration and approved by Ural State Medical University Ethical committee (№ 9, November 25, 2022).

Results: Before surgery, aggregation activity in ACF group was significantly reduced relative to the control (table 1), however, on the 1st day after surgery it increased to the control level, which probably can be the result of a higher platelet consumption (table 2). By 7th day, aggregation activity in ACF group decreased again relative to the control. In ETC group, aggregation did not differ from the control during entire study period, and no postoperative reactive thrombocytosis was detected.

Conclusion(s): Aceclofenac and etoricoxib do not suppress platelet aggregation activity in early postoperative period. Decrease in aggregation activity by 7th day when using aceclofenac can be important for reducing postoperative thrombotic risk in bone trauma.
