Background: There are few and contradictory reports on the potential effects of aging on coagulation parameters of healthy individuals.

Aims: To compare characteristics of coagulation in healthy controls stratified according to their age in ≤65 years old (yo) and >65 yo.

Methods: We included 50 individuals >18 to ≤65 and 25 individuals >65 yo. Individuals with cardiovascular diseases, previous thrombotic events, blood disorders or under treatment with antiaggregants and anticoagulants were excluded.
Kinetics of clot formation and fibrinolysis were evaluated using ROTEM. We recorded clotting time (CT); clot formation time (CFT); α angle; maximum clot firmness (MCF); and lysis at 60 minutes (LI60).
Thrombin generation was measured in platelet poor plasma by calibrated automated thrombogram. Coagulation was triggered by 1 pmol/L of tissue factor (TF) plus 4 µmol/L of phospholipids (PLs) for determining plasma procoagulant activity; or by 1 pmol/L TF or 4 µmol/L PLs for, respectively, PLs-dependent or TF- dependent microparticle-procoagulant activity. Lagtime; peak height; and endogenous thrombin potential (ETP) were determined.
Informed consent was obtained and the study was approved by Ethics Committee of Hospital Universitario La Paz.

Results: Individuals >65 yo showed a prolonged CT and CFT, and a diminished alpha angle. MCF was similar to that of ≤65, but LI60 was diminished (Figure 1).
Plasma from >65 yo had a lower coagulant capacity than plasma from ≤65 yo (Figure 2A). TF- (Figure 2B) and PL-associated (Figure 2C) coagulant capacity of MPs were also diminished.

Conclusion(s): Individuals >65 yo seemed to have a hypocoagulant profile evidenced by a prolonged CT that might be due to the reduced thrombin generated. Moreover, CFT with normal MCF might indicate a polymerisation disorder.
