Background: Patients with Pulmonary Embolism (PE) and cancer have higher in-hospital mortality (IHM) rates than those without cancer. The Pulmonary Embolism Severity Index (PESI) can categorize risk in PE; however, its usefulness in patients with cancer is limited in our setting

Aims: To determine whether patients with PE and active cancer, the presence of a PESI score ≥86 points is associated with higher IHM rates compared to those without cancer.

Methods: Retrospective analysis in patients with PE and a PESI ≥86 points, between 2008 and 2022. The occurrence of IHM was evaluated in patients with and without cancer. A multivariate logistic regression analysis was performed to assess the stability of results regarding IHM, mechanical ventilatory support (MVS) and use of vasopressor drugs (VPD), adjusted for age, gender, arterial hypertension, diabetes and active cancer.

Results: A total of 209 patients hospitalized for PE with a PESI ≥86 points were included in our analysis. Mean age 67 years (47.8% female). IHM was 11%. Compared to those without cancer, patients with cancer were younger (65 vs. 70 years; p< 0.05), had a lower prevalence of arterial hypertension (48% vs. 72%; p< 0.05) but more diabetes (19% vs. 8%; p< 0.05) Table 1. Within the patients with cancer, 23% were incidental PE and 12.6% had limitation of therapeutic effort. One-third of the different types of cancer were either lung or gastrointestinal. The multivariate analysis did not demonstrate that the presence of active cancer in patients with PESI ≥86 points is associated with higher IHM rates (12.7% vs. 8.4%; p=ns). Cancer patients had lower requirement for MVS (9.5% vs. 24.0%; p< 0.05), and VPD (11.1% vs. 22.8%; p< 0.05). Table 2.

Conclusion(s): In patients with PE and active cancer, the presence of a PESI score ≥86 points is not associated with higher IHM rates compared to those without cancer.
