PB0945 - Isolated deep vein thrombosis and isolated pulmonary embolism - are there any differences?

Background: Venous thromboembolism consists of two clinical entities - deep vein thrombosis and pulmonary embolism. However, in some cases these entities are isolated.

Aims: The aim of our study was to compare the characteristics of the patients with isolated deep vein thrombosis and the patients with isolated pulmonary embolism.

Methods: We performed a retrospective analysis of prospectively collected data of the patients with isolated deep vein thrombosis and with isolated pulmonary embolism who had visited thrombosis clinic in a tertiary hospital since January 2004 to January 2023. We collected the information about the personal and family history and the results of thrombophilia testing. We compared the differences between the two groups using chi square test.

Results: Of 1208 patients with venous thromboembolism, 497 had isolated deep vein thrombosis and 191 had isolated pulmonary embolism. There were no significant differences concerning the age and gender between the two groups ( 56±17 and 56±18 years; 46.9 % and 54.5 % women) and in the prevalence of obesity and thrombophilia. In the group with isolated deep vein thrombosis, the events were provoked significantly more frequently (27.2 % versus 12.6 %, P=0.0004) and significantly more events were provoked by leg infections or injury (20.7 % versus 2.1 %, P=0.017). Isolated pulmonary embolism was more frequently associated with covid (5.2 % versus 1.2 %, P=0.002) and with the use of hormonal contraception (13.6 % versus 7.6 %, P=0.022). The history of previous deep vein thrombosis was significantly more frequent in those with isolated thrombosis (22.5% versus 12 %, P= 0.005) and the history of previous pulmonary embolism significantly more frequent in those with isolated pulmonary embolism (11.5 % versus 5.8 %, P=0.014).

Conclusion(s): Deep vein thrombosis and pulmonary embolism may occur as isolated events and may differ in risk factors and in clinical characteristics of the patients.
