Background: Acquired thrombophilias significantly contribute to venous thrombosis(VT) risk. However, data in South India, a region with a known high VT burden, is limited.

Aims: To study the clinical profile of patients with acquired thrombophilia diagnosed with venous thrombosis.

Methods: We analyzed tests for lupus anticoagulant (LA), myeloproliferative neoplasm mutations (MPN), and paroxysmal nocturnal hemoglobinuria (PNH) from 3963 patients (4059 tests) done in our centre over the last 5 years(2019-2023). Among those with positive results (764), 151 had either arterial or venous thrombosis. Focusing on venous thrombosis alone, we included patients who either had clots at testing or developed them later and the data from these patients (n=93) was analyzed using spreadsheets.

Results: Of the 93 acquired thrombophilia patients identified, 100 had venous thrombotic events (LA-79, MPN-12, PNH 9). The median age of patients in the LA group was 44 years (6-63 years), in the MPN group was 51 years (16-86 years) and in the PNH group was 30yrs (20-56yrs). The youngest patient in the study (6years) was positive for LA while the oldest patient (86 years) was positive for MPN. The patients in MPN group were older when compared to the PNH group. The male:female ratio was 4:8 in MPN, 3:3 in PNH and 36:39 in LA. In the LA cohort, majority of the patients had either Deep Vein Thrombosis [DVT](54.46%) or Pulmonary Embolism (20.24%), in the MPN cohort, majority had thrombosis in splanchnic vein [SV] (66%) or Cerebral venous thrombosis [CVT](16.5%), and as in PNH cohort SV and CVT were equally prevalent (44.4% each).

Conclusion(s): Thrombotic manifestations varied across groups, showcasing the predilection for DVT in LA, SV involvement in MPN, and a balanced distribution between SV and CVT in PNH. However, the reason for this anatomic predilection still needs further research. Our study was limited by its retrospective nature.
