Background: Anti-phosphatidylserine/prothrombin antibodies (aPS/PT) could represent an additional marker for identifying patients with clinical suspicion for APS, especially when conventional aPL testing is negative or equivocal.

Aims: In this study our objective was to conduct a cost-utility analysis and Technology Assessment (TA) evaluating the overt benefit of introducing aPSPT in the routine laboratory diagnostic process for patients at high clinical suspicion for APS are still missing.

Methods: We conducted a survey with experts in the field of APS and the specific screening test procedure. The invitation to participate in the study has been sent to the mailing list of APS Consortium which aggregates more than 100 specialists/experts of APS from North-West Italy.

Results: When analyzing the responses of the survey (Graph 1), the assessment on the current situation of standard practice to diagnose APS (AS IS scenario) is substantially neutral in every domain of interest; in fact, graphics show that most of the lines interpolate the Zero point (no positive evaluation nor negative). On the contrary, the introduction of screening procedures (TO BE scenario) is considered as positive. The introduction of screening procedures is regarded as positive in terms of effectiveness and safety (for healthcare workers as well as patients); more specifically, the benefits in terms of “effectiveness” in the diagnosis of APS have been regarded as extremely positive. It is worth mentioning that there is also a positive and statistically significant association between the dimensions of Effectiveness of the screening test with the dimensions of Equity (r. 0,70; p 0.001) and Ethics (r. 0,67; p 0.001), respectively.

Conclusion(s): Our HTA analysis might support the implementation of aPS/PT testing in cases of high suspicion for APS, especially in the context of SLE.
