PB1039 - "The Differences in Plasminogen Activator Inhibitor-1 (PAI-1) and Tissue Plasminogen Activator (tPA) in Type 2 Diabetes Mellitus Patients"

Background: Increased levels of glucose in the blood in individuals with type 2 diabetes mellitus may trigger a sequence of pathological processes that contribute to the development of chronic problems, including cardiovascular disease, neuropathy, retinopathy, and nephropathy. A potential factor contributing to the heightened risk of severe problems is a disruption in the fibrinolysis system, involving Tissue Plasminogen Activator (tPA) and Plasminogen Activator Inhibitor-1 (PAI-1) as key components.

Aims: This study aimed to investigate the differences in tPA and PAI-1 concentrations among individuals with type 2 diabetes mellitus, comparing those with well-regulated and poorly regulated glucose levels.

Methods: This is an observational analytical cross-sectional study that was conducted from November 2023 to April 2024 in the clinical pathology laboratory of Dr. Soetomo Academic Hospital, Surabaya. A total of thirty patients were diagnosed with type 2 diabetes mellitus by internists who provided outpatient care at Dr. Soetomo Academic Hospital. Mann-Whitney U-Test was performed to compare the tPA and PAI-1 values in patients with controlled and uncontrolled type 2 diabetes mellitus. The laboratory examination of tPA and PAI-1 was performed with an enzyme-linked immunosorbent assay. The criteria of controlled and uncontrolled type 2 DM patients using HbA1C laboratory parameters.

Results: The difference between PAI-1 parameters in controlled and uncontrolled type 2 DM patients using the Mann Whitney U-Test obtained a statistical Z value of -3.423 with a p-value of 0.001 (p < 0.05). The differences in tPA parameters in controlled and uncontrolled type 2 DM patients using the Mann-Whitney U-Test obtained a statistical Z value of -2.241 with a p-value of 0.025 (p < 0.05).

Conclusion(s): There are significant differences in PAI-1 and tPA parameter values between controlled and uncontrolled type 2 DM patients.
