Background: Platelet products (PLT) play a crucial role in hemostatic resuscitation of bleeding trauma patients. In the US, PLT can be manufactured in different resuspension/storage media (plasma or platelet additive solution (PAS)). PLT are currently discarded after 5-7 days of room temperature storage. Cold storage (CS) of PLT may extend shelf-life while still preserving hemostatic function. Comparisons between collection medium and storage conditions of PLT are limited and could support expanded use of CS-PLT.

Aims: We aim to examine how resuspension medium impacts hemostatic function in PLT over storage in cold (CS) and room temperature (RT) conditions.

Methods: PLT were collected via standard apheresis using the Trima Accel® 7 (Terumo; “Trima”) and suspended in either plasma and preservative (n=9) or PAS (n=9) and preservative. Units were sampled post-collection, then stored at 4°C (CS) for 21 days or 22°C (RT) for 7 days. Assessment of hemostatic function was accomplished via light transmission aggregometry (LTA), rotational viscoelastometry (ROTEM), thrombin generation, and a microfluidic model of hemostatic resuscitation of induced dilution or platelet inhibition (via 6.4uM ticagrelor). Statistical comparisons were made using unpaired t-tests (alpha=0.05).

Results: Plasma PLT had a smaller percent change over storage time for each hemostatic parameter compared to their PAS counterparts (Figure1). CS-PLT had minimal loss of function for LTA and ROTEM parameters but were still similar at d21 storage compared to d7 RT-PLT. In the microfluidic model, donor and recipient platelets were incorporated into clots and PAS and plasma PLT had similar surface area deposition in hemostatic clots achieved at arterial shear (3500/s) (Figure2).

Conclusion(s): CS-PLT largely maintain hemostatic function over 21 days compared to their RT counterparts after 7 days. Plasma storage induced somewhat less change in hemostatic function. Extended PLT storage duration with CS-PLT could solve the problem of blood product shortages across the country for patients with bleeding.
