Background: Several studies showed that people with adequate health literacy are more likely to conduct healthy behavior, have less hospitalization and exhibit better health outcomes. Assessing health literacy will contribute to patient education or health intervention; however, few research examines health literacy of people with haemophilia (PwH).

Aims: To evaluate the current level of health literacy and information channel preferences among adult PwH.

Methods: In total 1765 adult PwH were recruited from hemophilia treatment centers throughout the country to participate in web-based survey of health literacy and information preferences from September to October 2023. Haemophilia-focused health literacy consists of three dimensions: foundational disease knowledge, reading comprehension of health materials and factor dose calculation based on Package Insert. Data analysis was based on descriptive statistics and post-hoc significance testing.

Results: The average age of adult PwH was 33.8±10.1 years, of whom 86.8% were diagnosed with haemophilia A, 64% had severe disease ( < 1% normal) and 61.3% received prophylaxis regimen. The proportion of participants aged 18-34 years (n=1010) deemed to have demonstrated health literacy in foundational knowledge, reading comprehension and dose calculation was 33.6%, 64.6% and 21.1%, respectively. Among those ≥35 years (n=755), 32.7%, 59.7% and 23.6% demonstrated the three dimensions of haemophilia-focused health literacy, respectively. Reading comprehension was the only dimension where there was a significant difference between the 18-34 and ≥35 subgroups (64.4% vs. 59.7%, p< 0.05). The top choices of channels through which participants acquire health information, ranked in the order of preference, are social media (non-short-form video) (preferred by 27.1% of all participants), healthcare professionals (18.2%), traditional media outlets (16.3%) and patient organizations (15.9%).

Conclusion(s): Adult hemophilia patients who participated in the survey demonstrated a moderate level of essential disease knowledge. This was the first study of disease-relevant health literacy and numeracy among adult haemophilia patients in China.
