Background: There is emerging evidence that the antithrombotic metalloprotease ADAMTS13 is decreased in sepsis, a life-threatening disorder that is accompanied by endothelial activation. However, it remains unknown whether loss of ADAMTS13 activity in sepsis is correlated with age, sex or race.

Aims: To determine ADAMTS13 activity as a novel biomarker in sepsis and test its association with age, sex and race.

Methods: Thirteen citrated healthy controls (4 males, 9 females, median age 30) and forty sepsis patient samples (20 males, 20 females, median age 64) collected within 24 hours of ICU admission were obtained from the Sepsis ClinicAl Resource And Biorepository (SCARAB) Study at Vanderbilt University Medical Center. Citrated pooled normal plasma (PNP) was acquired commercially, and eight acute and six remission TTP samples were previously reported. Plasma samples were recalcified before analyzing ADAMTS13 activity using our recently reported assay Cattle-FRETS71 (Barton et al. JTH 2023). Samples were assayed in duplicates and analysis was performed by SAS.

Results: The septic group’s ethnic demographics included White (85%) and Black (7.5%) patients. The mean ADAMTS13 activity was reduced in the sepsis group (50.85±2.9%, p< 0.0001, Figure 1) compared to controls (100.1±4.8%). However, there was no significant difference between males and females within the sepsis group (50.06±4.5% vs 51.66±4.5%, p=0.80). Unexpectedly, there was no significant correlation between age and ADAMTS13 in the sepsis group (r=-0.36, p=0.02). As expected, the mean ADAMTS13 activity in acute and remission TTP groups was significantly decreased compared to controls, (2.86±4.1%, p< 0.0001) and (23.57±4.8%, p< 0.0001), respectively. In this small cohort, ADAMTS13 activity was lower in deceased versus alive sepsis patients (43.87±7.1% vs 52.60±3.5%, p=0.28).

Conclusion(s): ADAMTS13 activity is significantly reduced in sepsis but does not differ by age, sex or race. ADAMTS13 may be lower in patients who died. Future studies will test the association between ADAMTS13 and organ failures in sepsis.
