Background: Cancer growth develops a subclinical hypercoagulable state and hypercoagulability is also a key contributor in cancer progression beyond cancer-associated thrombosis. Coagulation proteins are expressed by cancer cells and have multiple functions in tumor progression and metastasis, although current knowledge is rather limited.

Aims: To analyze the expression of hemostasis-related genes in urological tumors and to validate their expression in plasma from patients.

Methods: We analyzed in silico the expression level of 21 coagulation factors and hemostatic proteins in urological tumor specimens (bladder urothelial carcinoma, renal chromophobe, renal clear cell carcinoma, renal papillary cell carcinoma and prostate adenocarcinoma) and control tissues using the online tool TNMplot, which combines public gene expression data obtained by RNA-seq from tumor specimens, metastases and normal tissue from the GEO, GTex, TCGA, and TARGET databases. Additionally, we recruited 84 urological cancer patients (45 bladder, 14 kidney and 25 prostate cancer) and 63 controls, and quantified in plasma 11 of these markers of coagulation, natural anticoagulants and fibrinolysis (fibrinogen, FVIII, FVII, FXIII, FVW, protein C, protein S, antithrombin, plasminogen, PAI-1, α2-antiplasmin).

Results: Compared to healthy tissue, we found a dysregulation in tumor specimens of all hemostatic proteins studied, being the up- or downregulation tumor specific (Table 1). In plasma of cancer patients, we found an upregulation of FXIII, antithrombin and alpha-2-antiplasmin compared to controls (Figure 1). Concerning specific tumor types, we found an upregulation in FVII (prostate), FXIII (prostate), antithrombin (renal and prostate), PAI-1 (prostate) and alpha-2-antiplasmin (bladder and renal).

Conclusion(s): Active urological cancer patients display a dysregulation in hemostatic markers, both locally and at systemic level, which differ among tumor types. The expression at the tumor site rarely matches the systemic levels, suggesting a different contribution of these proteins in tumorigenesis and the trigger of cancer-associated thrombosis.
