Background: Hematopoietic stem cell transplantation (HSCT) is a vital therapy for various hematopoietic diseases, and platelet recovery is crucial to the prognosis of HSCT patients. However, previous studies have predominantly focused on platelet count recovery, leaving the molecular signature of platelets in HSCT patients and its alterations during transplantation complications such as acute graft-versus-host disease (aGVHD) largely unknown.

Aims: This study aims to unveil the molecular dynamics of platelets after HSCT and explore their significance in diagnosing aGVHD.

Methods: Peripheral blood samples were collected from healthy donors (HD), HSCT patients without complications, and aGVHD patients with informed consent. Bulk RNA sequencing was performed on platelets, and bioinformatic analysis was conducted using R (v4.2.1). All procedures were approved by the Ethics Committee of the Institute of Hematology, Blood Diseases Hospital, Peking Union Medical College, and Chinese Academy of Medical Sciences.

Results: The study observed dynamic changes in the molecular signature of platelets during platelet recovery in HSCT patients. Notably, alterations in immune signatures were particularly significant: the expression of innate immune-related genes gradually declined, while the expression of adaptive immune-related genes increased after HSCT. Furthermore, these immune signatures were distinct from aGVHD patients. Specifically, aGVHD patients exhibited higher expression of innate immune-related genes and lower expression of adaptive immune-related genes. Moreover, platelets from patients with more severe aGVHD expressed lower levels of adaptive immune-related genes.

Conclusion(s): This study systematically revealed the dynamics of platelet immune features after HSCT and their changes in aGVHD patients. These findings suggest that the molecular pattern of platelets may play a crucial role in the diagnosis of aGVHD and its severity.
