Background: Retinal vein occlusion (RVO) is the second most common retinal vascular disease. Limited number of studies on risk factors and outcomes for RVO have been conducted.

Aims: To investigate risk factors of RVO and factors associated with poor outcomes in RVO treated with intravitreal anti-vascular endothelial growth factor (anti-VEGF).

Methods: This was a retrospective case-control study of patients with RVO who visited the outpatient ophthalmology clinic of Chiang Mai University Hospital and received anti-VEGF between January 2012 and December 2021. The control group consisted of patients who visited the clinic during the same period and were diagnosed with other eye problems with normal visual acuity. The data collected included baseline characteristics, medical history, medication, visual acuity, glycemic status, lipid profile, and blood count. Poor outcomes were defined as disease progression or recurrence. Factors with a p-value < 0.01 from univariable analyses were included in multivariable analyses.

Results: This study included 700 patients (350 in each group). Statins significantly decreased the risk for RVO (adjusted odds ratio (aOR), 0.34; 95% confidence interval (CI), 0.13-0.86; p=0.023). In contrast, significant risk factors for RVO included age (aOR, 1.04; 95%CI, 1.03-1.06; p< 0.001), diabetes (aOR, 3.02; 95%CI, 1.68-5.40; p< 0.001), hypertension (aOR, 2.82; 95%CI, 1.75-4.56; p< 0.001), smoking (aOR, 3.92; 95%CI, 1.11-13.80; p=0.033), and anemia (aOR for each g/dL of decreasing hemoglobin, 1.43; 95%CI, 1.09-1.86; p=0.008). The prevalence of poor outcomes of RVO was 32.8%. Diabetes (aOR, 1.77; 95%CI, 1.08-2.91; p=0.022) and the diagnosis of central RVO (aOR, 1.93; 95%CI, 1.09-3.42; p=0.023) were significantly associated with poor outcomes.

Conclusion(s): Statins are found to be a protective factor against RVO, while age, diabetes, hypertension, smoking, and anemia increase the risk of RVO. Diabetes and the diagnosis of central RVO, compared to branch RVO, are related to poor outcomes in RVO treated with anti-VEGF.
