Background: We developed venous thromboembolism risk assessment and integrated care system (DeVTEcare), linking with electronic health record, can to timely track the VTE and VTE-related death in hospital, which validated with good identification in our prior study.

Aims: The present study aimed to investigate the in-hospital VTE and its-related death over time across different clinical setting.

Methods: The patients aged over 18 years admitted to Sixth Medical Center of the Chinese People's Liberation Army General Hospital from January 1 2021 year to December 31, 2023, were included into this analysis. The in-hospital VTE and its-related death were calculated and the underlying contributed factors were analyzed. We explored the association between the top complexity level (4th) , according to a four-level classification to rate surgical complexity, and in-hospital VTE in Department with highest rate of VTE event.

Results: There were 106865 in-hospital patients (mean age, standard deviation, 55,19，years old, 54, 55.5% male). Among them, there were 33 patients with in-hospital pulmonary thromboembolism (PE), 1166 patients with deep venous thromboembolism (DVT), and 51 deaths related to VTE.

The rate of VTE of Departments of Surgery (including Neurosurgery, Orthopedics, Chest surgery, General surgery) was much higher than Departments of Internal Medicine (including Tumor, Gastroenterology department, Cardiology, Respiratory) (343/17729, 1.93% vs. 183/21181, 0.86%, p< 0.05).

The annual rate of in-hospital VTE of Neurosurgery ranged between 6.40% to 11.40%, followed by 5.22%-6.49% of Orthopedics, and 1.64%-3.12% of General surgery (p < 0.05, Table 1). The Peri-operative VTE of Neurosurgery, stratified by the complexity of surgery was in Table 2.

Conclusion(s): The DeVTEcare system, embedded into the electronic health record, timely monitored in-hospital VTE across different clinical setting, highlighting the need for effective peri-operative VTE prophylaxis.
