PB0432 - Smart technology facilitating patient-centered venous thromboembolism management : pilot assessment for the adherence

Background: Treatment adherence is crucial to patient-centered self-management. Mobile health technology may affect venous thromboembolism (VTE) patients' thromboprophylaxis adherence.

Aims: This study investigated how mobile health-facilitated behavioural modification affects thromboprophylaxis adherence in VTE or moderate-to-high-risk patients.

Methods: This was a pilot study of VTE or moderate-to-high-risk VTE patients over 18 years old enrolled in Chinese PLA General Hospital between August 25 and December 20, 2023. Patients received mobile VTE Application (mVTEA) for self-management, which automatically sent anticoagulant-taking interactive reminders. The Morisky Medication Adherence Scale (MMAS-8) measured medicine adherence, whereas the Beliefs about Medicines Questionnaire (BMQ)-Specific assessed patients' anticoagulant beliefs. The real-time anticoagulant adherence was analyzed at 1 month. Trial registration: www.chictr.org.cn, no. ChiCTR2200063206.

Results: Out of 88 patients (age: 63.85±15.41 years, female: 23 [26.14%]), 52 (2 [3.85%] moderate-risk, 7 [13.46%] high-risk, and 43 [82.69%] VTE) registered with mVTEA. And, 43/52 (82.69%) were on anticoagulants. 45/52 (86.54%) patients completed MMAS-8 and BMQ score questionnaires. Good, medium, and poor adherence were 13/45 (28.89%), 16/45 (35.55%), and 16/45 (35.55%). Poor adherence was most often caused by “forgetting drug-taking” or “missing dosage”. With BMQ scores of 17.84±3.48 for necessity and 14.46±3.58 for concerns, patients with the necessity issues most often said, “I worry about becoming dependent on my anticoagulants” or “Because I don't like the side effects”.The mVTEA interactive drug reminder check-in rate was over 80% for more than half of patients with good baseline adherence. Even for those with poor baseline adherence had check-in rate of 75%. All patients using mVTEA took anticoagulants on time during one-month follow-up.

Conclusion(s): The smart technology, mVTEA, engaged patients in self-care through behavioral modification and maintained anticoagulant use. Meanwhile, individualized patient education may further improve adherence.
