PB1405 - Risk Factors and Mortality Associated with Venous Thromboembolism in the Elderly White, Black and Asian Patients with Chronic Lymphocytic Leukemia in the United States

Background: Data are limited for venous thromboembolism (VTE) in elderly patients with chronic lymphocytic leukemia (CLL).

Aims: To determine risk factors and mortality associated with VTE in White, Black, and Asian CLL patients.

Methods: The United States SEER-Medicare database (2000 to 2015) was used for CLL patients ≥ 65 years. Race was defined as White, Black or Asian. VTE was defined as deep vein thrombosis or pulmonary embolism during the study period. Logistic regression was used to examine VTE risk factors and Cox proportional regression was used to evaluate the effect of VTE on mortality.

Results: Among 34,075 of CLL patients, VTE was diagnosed in 11.6% of 31,395 White, 14.6% of 2,062 Black and 6.3% of 618 Asian patients. Odds of having VTE was, ORa=1.2, 95% CI, 1.0-1.4 for Black patients and ORa=0.5, 95% CI, 0.4-0.7 for Asian patients compared to White patients. VTE risk factors in White patients were the same as overall results. These included anemia (OR= 2.0, 95% CI, 1.9-2.2), chemotherapy (OR=1.3, 95% CI, 1.2-1.5), hypertension (OR=1.8, 95% CI, 1.6-2.0), heart failure (OR=1.5, 95% CI, 1.4-1.6), obesity and chronic lung disease (OR=1.3, 95% CI, 1.2-1.4), kidney disease (OR=1.2, 95% CI, 1.1-1.3), diabetes and hyperlipidemia (OR=1.2, 95% CI, 1.1-1.2), and myocardial infarction (OR=1.1, 95% CI, 1.0-1.2). VTE risk factors for Black patients included anemia (OR= 3.0, 95% CI, 2.1-4.3), chemotherapy (OR=1.9, 95% CI, 1.1-3.0), hypertension (OR=2.1, 95% CI, 1.3-3.3) and heart failure (OR=2.0, 95% CI, 1.5-2.7) and for Asian patients, anemia (OR= 5.1, 95% CI, 1.4-18.3) and heart failure (OR=4.9, 95% CI, 2.1-11.4). VTE was associated with mortality (HRa=1.05, 95% CI, 1.00-1.09) in overall population and in White patients (HRa=1.06, 95% CI, 1.01-1.10) but not in Black and Asian patients.

Conclusion(s): VTE risk factors were not the same in White, Black, and Asian CLL patients.
