Background: Lymphatic malformations and venous malformations are caused by abnormal development of the vascular system. For low-velocity vascular malformations, they are usually caused by abnormalities of the lymphatic and venous systems. In the female twins we reported, both had low-velocity vascular malformations, but one had a lymphatic malformation and the other a venous malformation, which is very rare.

Aims: The report describes a rare case of low-velocity lymphatic malformation and venous malformation in a pair of 13-year-old twins, and provides experience for better understanding and treatment of these rare vascular malformations.

Methods: The older of the twins, it was confirmed by magnetic resonance imaging (MRI) that she had lymphatic malformation. Hemodynamic evaluation showed that the vascular malformation was of low flow type. Bleomycin injection was used to treat the lymphatic malformation. In the younger twin, MRI results showed that she had a low-velocity venous malformation, which was treated with bleomycin combined with polydocarol hardener.

Results: Lymphatic and vascular malformations were significantly reduced and pain symptoms disappeared.

Conclusion(s): For rare vascular malformations, we should remain on high alert, explore appropriate treatment strategies, and strengthen multidisciplinary cooperation to provide better diagnosis and treatment options. Individualized treatment options and multidisciplinary collaboration play an important role in the management of rare diseases and also help to improve patients' quality of life and long-term prognosis.
