PB1421 - Trends in the incidence and outcomes of thrombotic events in renal cell carcinoma in the Asian population in USA - National Inpatient Database Study 2016-2020

Background: Although renal cell cancer (RCC) patients are known to be at a higher risk of thrombosis, there is a paucity of data about the outcomes of venous thrombosis among the Asian population with RCC in the USA.

Aims: Assess the trends in incidence and outcomes of thrombotic events in Asian patients with RCC in USA.

Methods: Retrospective analysis of the National Inpatient Database from 2016 to 2020 was performed using ICD 10 codes. Logistic regression and linear regression were performed to find an association between the presence of thrombotic events like pulmonary embolism (PE), deep vein thrombosis of limbs (DVT), and portal vein thrombosis (PVT) and mortality and length of stay (LOS). Results were adjusted for age, gender, race, smoking, alcohol, pregnancy, hormone use, end-stage kidney disease, and thrombophilias.

Results: Out of 2034 RCC patients (mean age 62.7 years, 36% females), 93 (4.6%) developed venous thrombosis. There were zero inpatient deaths reported among RCC patients with venous thrombosis in 2016-18 while 4.3% of the patients died in 2019-20. Data from 2019 (OR 6.1, p value 0.07, CI 0.5-57.9) and 2020 (OR 5.3, p value 0.17, CI 0.9-43.2) showed that RCC patients with PE had a higher risk of inpatient mortality than those without PE. DVT and PVT did not show a significantly increased risk of mortality. Data from 2016 (OR 1.9, p value < 0.05, CI 1.3-2.9) and 2019 (OR 3.2, p value < 0.05, CI 1.3-7.9) showed that RCC patients with DVT had a significantly increased LOS. Although PE was also associated with an increased LOS across all years, it was not statistically significant.

Conclusion(s): Inpatient data on the association between PE, DVT, and PVT and inpatient mortality and LOS is not uniform across 2016-20 among Asians with RCC. The occurrence of DVT in Asian patients with RCC can significantly increase their LOS.
