PB0462 - Anticoagulants In The Treatment of Primary Antiphospholipid Syndrome In Pregnant Women: A Systematic Review

Background: Antiphospholipid Syndrome (APS) is an autoimmune thrombophilic condition that mark by the presence in blood of antibodies that recognize and attack phospholipid-binding proteins, rather than phospholipid itself. Women with APS and history of thrombosis should receive full heparin
anticoagulation throughout pregnancy.

Aims: To evaluate the use of anticoagulants on pregnant women with primary antiphospholipid syndrome.

Methods: We performed a systematic literature search from PubMed with the search terms: ‘Primary antiphospholipid syndrome’ AND ‘Pregnancy’ AND ‘Anticoagulant’ AND ‘Fetal loss’. The type of study designs included in the review were Randomized Control Trial (RCT), Cohort, and Clinical Trial. The study was carried out according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The Newcastle Ottawa Scale (NOS) and the Jadad Scale was used as the basis for this review.

Results: Through the inclusion criteria, 3 journals were obtained according to the Randomized Control Trial (RCT), Cohort, and Clinical Trial research description. Based on the Newcastle Ottawa Scale (NOS) and the Jadad Scale, 2 studies were rated good and 1 studies were fair. Based on this research, it was found that in patients with primary antiphospholipid syndrome, the use of anticoagulants with and without low dose aspirin (LDA) resulted in a high live birth rate ranging from 82-86.25%.

Conclusion(s): From this research it was found that in patients with primary antiphospholipid syndrome, anticoagulant use resulted in a high live birth rate. Anticoagulant used in the 3 journals were LMWH. 2 journals studied LMWH in combination with LDA. 1 study compared the use of Bemiparin with Aspirin and found that Bemiparin resulted in a higher live birth rate with a mean difference of 0.141 (95% CI 0.08 - 0.274). However further multi center RCT is needed to confirm the result.
