Background: Cardiovascular diseases (CVD) and cancer are the most common causes of death worldwide. Breast cancer is the most prevalent cancer in women with the highest mortality. Breast cancer patients under doxorubicin (DOXO)-based chemotherapy present coagulation abnormalities. Thrombin generation test (TGT) may be useful in evaluating the hemostatic status of these patients.

Aims: To evaluate the performance of TGT according to cardiovascular risk in breast cancer patients undergoing DOXO-based chemotherapy.

Methods: Blood samples were collected from 80 women with breast cancer at different times, before (T0) and up to 7 days after the DOXO-chemotherapy (T1). Patients were classified according to the Global CVD risk. Two parameters of TGT were used: endogenous thrombin potential (ETP, nM•min) and Peak of Thrombin (nM) to evaluate the hemostatic status. TGT was performed by CAT method. The study was approved by local Ethics Committee and is in accordance with the Declaration of Helsinsk.

Results: Patients were classified as low- (32,4%), intermediate- (24,3%) and high-risk (24,3%) to CVD. The mean DOXO-dose (380, IQ 360 – 417 mg/m2) and BMI (29,2 ± 5,7 kg/m2) were similar between groups (p>0.05 for both). Age (years) was higher in high-risk group (62,0 ± 7,5) than intermediate-risk (52,5 ± 8,0) and low-risk (39,9 ± 10,05, p< 0.001 for all). No difference between the groups regarding the TGT parameters and CVD risk was observed. However, ETP and Peak of Thrombin were higher over the time (T1>T0 for the three groups) (Table 1 and 2).

Conclusion(s): These findings suggest that DOXO-chemotherapy increases the hypercoagulable status in breast cancer patients independently of CVD risk.
