Background: An association between non-O blood type and increased Venous thrombo-embolism risk in cancer patients has been reported recently; but it is unclear whether ABO type has impact on chemotherapy induced-hypercoagulability or thrombosis risk following chemotherapy.

Aims: To assess coagulability and examine the relationship between ABO groups, thrombosis risk and hypercoagulability state in female cancer pre and post chemotherapy.

Methods: Twenty-eight female cancer patients planned for chemotherapy were recruited and subdivided into O and non-O groups. VTE risk factors and Khorana score (KS) were assessed. Coagulation profile was examined prior to and after the first cycle of chemotherapy, using Thromboelastography (TEG® 5000). Five different TEG parameters were compared pre and post-chemotherapy using student t-test. Chemo-induced hypercoagulability was assigned if ≥2 parameters changed towards hypercoagulability following chemotherapy and hypercoagulability pre-chemotherapy was assigned in comparison to control. TEG parameters were compared between non-O and O blood groups before and after chemotherapy using Chi-Square Tests.

Results: Twenty-eight patients (18 non-O, 10 O-blood group) were analyzed. Patients' characteristics and VTE risk factors are provided in table 1. Data shows 72.2% (13/18) in the non-O blood group were hypercoagulable post-chemotherapy versus 27.8% (5/18) for the O blood group. No significant association was observed between blood type and chemo-induced hypercoagulability state (p = 0.6). However, following chemotherapy, patients with non-O blood groups showed significant reduction in R-time, K-time (p=0.03 ; 0.02 respectively); with a trend towards hypercoagulability in alpha-angle (P=0.09) versus pre-chemo (Table 2). No significant association was found between ABO and KS (p=0.5). Two patients (11%) developed VTE within 6-months of starting of chemotherapy; both were non-O group and were hypercoagulable on TEG pre-chemotherapy.

Conclusion(s): ABO blood types may not predict hypercoagulability state but specific TEG parameters have the potential to distinguish between O and non-O patients’ chemo-induced hypercoagulability and thrombosis risk. Larger studies are needed to verify these findings.
