OC 31.1 - Thrombosis Complications in Postpartum Women: A Comparative Analysis of Cesarean Section and Vaginal Delivery in Armenia (2016-2023) – Insights from a Single-Center Study

Background: Thrombosis remains a significant concern in the postpartum period, and its incidence varies between different modes of delivery.

Aims: This study presents a comparative analysis of thrombosis complications following cesarean section (CS) and vaginal delivery in Armenia, focusing on the years 2016-2021 and drawing insights from a comprehensive single-center study.

Methods: A retrospective cohort study was conducted, encompassing 51944 postpartum women who underwent either cesarean section or vaginal delivery at Erebouni medical center, Armenia. Patient records from 2016 to 2021 were meticulously reviewed to identify cases of deep vein thrombosis (DVT) and pulmonary embolism (PE). Demographic, medical, and obstetric data, as well as annual reports were collected for a detailed analysis, with a particular emphasis on the unique regional context. Studies on thrombophilia or recurrent DVT were excluded, and the search was restricted to prospective studies when assessing the incidence of DVT and PE. Pooled relative and absolute risks were estimated with random effects models.

Results: The overall incidence of thrombosis complications in the study population was 4.1 per 1000 cases. Notably, cesarean section exhibited a higher prevalence of thrombotic events compared to vaginal delivery (8.3 per 1000 cases vs. 2.2 per 1000 cases, p < 0.05). Advanced maternal age, obesity, and a history of thrombotic disorders emerged as significant risk factors, highlighting the need for tailored preventive strategies.

Conclusion(s): Thrombosis complications in the postpartum period are a critical consideration and this study underscores the higher risk associated with cesarean section in the Armenian context. The findings advocate for increased awareness among clinicians, emphasizing the importance of risk stratification and the implementation of targeted thromboprophylaxis measures. The insights gained from this single-center study contribute to the refinement of obstetric practices and warrant further investigation into regional variations.
