PB0514 - Joint health in patients with hemophilia: data from real-life practice in Colombia

Background: Arthropathy is the main morbidity of hemophilia. Periodic evaluation of these patients allows to adjust treatment and reduces joint damage. Evidence from the real world in countries like Colombia has not been widely studied

Aims: to characterize the joint health and prophylaxis of patients with hemophilia treated in a multicenter institution in Colombia

Methods: a cross-sectional study of patients with hemophilia aged 1 or more since February 2022 until August 2022. Haemophilia Activities List (HAL) and pediatric HAL (PedHAL) were applied. The Hemophilia Joint Health Score (HJHS) and Functional Independence Score in Hemophilia (FISH) were evaluated. The analysis of quantitative variables is presented as a median and interquartile range (IR); qualitative variables are summarized with frequency measurements and %.

Results: 225 patients were included, 72 under 18 years of age and 153 adults; 20.4% of the cases had mild hemophilia, 16.9% moderate, and 62.7% severe. Of the total population 28.9% received treatment on demand, 24% primary prophylaxis, 14.7% secondary and 32.4% tertiary. In minors, 100% obtained pedHAL results without functional limitation. In adults, 45.8% showed functional limitation; according to HJHS, 52.2% presented joint health risk, and in FISH, 9.5% obtained mild disability, 0.9% moderate, and 79.6% without disability. Adults from 29-59 years showed the worst results for HAL and HJHS, while for FISH it was the ≥60 group. At the time of the evaluation, 28% presented hemarthrosis, 12.6% presented hemarthrosis twelve months before the evaluation and 22.7% of these were traumatic. Finally, 52.9% of the patients presented hemophilic arthropathy, with a median number of involved joints of 1 (IQR 0-3).

Conclusion(s): in the current study, joint health showed no limitation or functional disability that interfered with daily activities in pediatric patients who did not have access to prophylaxis. However, adult patients still suffer hemophilia-related complications like arthropathy.
