Background: Lupus anticoagulant (LA) are types of immunoglobulins targeting the coagulant factors and blood clotting process. LA can lead to the prolong of activated (aPTT) in laboratory tests but do not cause bleeding in most cases. In the previous reports, LA were tested positive in some of the pediatric viral respiratory patients. And such positive LA in children were reported more than before in the post-pandemic of COVID-19, especially in 2023. However, the leading cause has not been clearly clarified.

Aims: We intend to analyze the clinical characteristics and laboratory test results of pediatric LA positive patients after the outbreak of COVID-19.

Methods: We collected all the information of positive LA pediatric patients that hospitalized in The Second West China Hospital of Sichuan University in 2023, including age, sex, clinical diagnosis, laboratory examination such as aPTT, LA1/LA2, and virus infectious. Positive rate of LA in children were analyzed and compared with results before outbreak of COVID-19.

Results: Different levels of positive LA with prolonged aPTT were found in 34 (65.3%) pediatric patients among total 52 cases who did the LA test; the median age was 5.5 (2 to 14). 19 of these were diagnosed as respiratory tract infection, of whom 6 patients (17.6%) were diagnosed as mycoplasma pneumonia infection. Others include EBV infection (n=4, 11.8%), influenza virus (n=3, 8.9%), rhinovirus (n=1, 2.9%), adenovirus (n=1, 2.9%) norovirus (n=1, 2.9%) and so on. (Table 1). The positive rate of LA in children before COVID-19 (2018-2022) was 29% (34 in 117).

Conclusion(s): Viral respiratory infection has a strong correlation with positive LA in children, in which mycoplasma pneumonia is the leading cause of infection. Such viral infection always leads to strong LA in children while weakly LA is the major result in adult LA tests. Moreover, a significant increase of strong LA after COVID-19 has been observed.
