Background: Thromboangiitis obliterans (TAO) is a rare vascular thrombotic disease characterized as an inflammatory disease mainly affecting young male smokers. However, rural-urban based differences in TAO prevalence and limb amputation rate in a real-world datasets remain unclear.

Aims: In this study, we aim to confirm TAO prevalence and limb amputation rate for inpatients with TAO according to rural and urban areas in a real world dataset.

Methods: We assess rural-urban differences in a large, administrative Nationwide Inpatient Sample (NIS) 2016-2019. The diagnosis of TAO and comorbidities were identified by ICD-10-CM codes. Univariable comparisons between rural-urban were assessed by Mann-Whitney. Multivariate logistic models were used to analysis associations between rural-urban and TAO prevalence, limb amputation, adjusted with demographics characteristics (eg. age, sex, race/ethnicity), comorbidities (eg. obesity, dyslipidemia, hypertension) and behavior disorders (eg. tobacco, alcohol abuse).

Results: In this cohort study of patients with TAO (N=824), 175 (21.2%) of them were from rural area and 649 (78.8%) of them were from urban area. TAO prevalence rate showed 53.9% higher in rural area (rural: 15.02%ooo urban: 9.76%ooo, p< 0.001), with adjusted odd ratio (OR) at 1.34 (95% CI, 1.12-1.59; p< 0.001). The cumulative limb amputation rate was up to 28.0% in rural area and 28.9% in urban area (p=0.815), with adjusted OR at 1.26 (95% CI; 0.84-1.89; p>0.05) for patients in rural area.

Conclusion(s): Patients in rural area showed significant higher rate of TAO prevalence. The cumulative limb amputation rates were up to ~28% both in rural and urban areas. The rural-urban based disparities and high rate of limb amputation spur further studies on subgroup analysis, mechanism, health equity and multidisciplinary management for patients with TAO.
