PB0069 - The assessment of the role of ROTEM in thrombosis risk evaluation of the severe form of COVID 19 patients

Background: Thrombosis is a frequent complication in COVID-19 evolution due to many pathogenetic factors especially due to hypercoagulability status. Rotational thromboelastometry (ROTEM) that assessed hemostasis could be important in the evaluation of the hypercoagulation status of COVID-19 patients.

Aims: This study aims to evaluate the correlation between ROTEM parameters, inflammatory markers, thrombophilic conditions, and clinical outcomes in severe form of COVID-19 patients

Methods: : This retrospective study enrolled 100 patients with a severe form of COVID-19 admitted in the intensive care unit (ICU) and 20 patients admitted to ICU negative for COVID-19. We assessed the relationship between the parameters of three ROTEM® tests (INTEM, EXTEM, and FIBTEM) and levels of CRP, interleukin-6, PROTEIN C, PROTEIN S, antithrombin III level, antifosfolipidic status and clinical outcomes.

Results: ROTEM indicated hypercoagulability in COVID-19 patients in all the tests performed. The levels of all inflammatory markers were significantly higher in COVID-19 patients. Patients with high grades of inflammation induced by a severe form of COVID-19 clot formation acceleration, high clot strength, and reduced fibrinolysis all of them characteristic of hypercoagulability status. Moreover, the FIBTEM parameters are correlated not only with inflammatory biomarkers and severity of pulmonary involvmentș high correlation was obtained with severity of protein S deficiency.:

Conclusion(s): ROTEM tests, especially FIBTEM parameters are very important in the evaluation of thrombosis risk in COVID-19 patients and are highly correlated with severe inflammation present in COVID-19 patients
