Background: Biospecimen banking is a growing field in health science translational research. Short to moderate-term storage of serum samples has been shown to be a suitable source for novel cardiovascular (CV) biomarker discovery. However, the effect of longer storage (>30 years) on CV marker stability is unknown. We are currently studying inosine and hypoxanthine as markers of MI and their relationship to established markers.

Aims: To study the stability of cardiac biomarkers during long-term biospecimen banking

Methods: : Serial serum samples (n=60) collected from 20 patients receiving tissue plasminogen activator(tPA) during the Thrombolysis in Myocardial Infarction Study Group phase II trial (TIMI II) conducted between 1986-1988 were obtained from the NHLBI BioLINCC biorepository. These samples were stored at-85°C. High sensitivity Troponin-I (hs-TnI) was analyzed using a Seimens Advia Centaur platform. Acute myocardial infarction (AMI) diagnosis was defined as hs-TnI >500 ng/L and CK >ULN per local reference ranges by study site. Mean differences and concordance between markers were evaluated at pre- and 4-, and 8-hour post-tPA therapy time points.

Results: The average age was 56 years, 85%were men, and 80% were whites. Both mean CK and hs-TnI levels were significantly (p < 0.001) elevated from pre-tPA time points with similar trends (Table). There was a95% concordance of CK and TnI values by AMI definition.

Conclusion(s): Despite long-term storage of serum samples, concordance between baseline and post-reperfusion levels was high, with little variability across time points for hs-TnI and CK. These data support the use of adequately stored samples for developing novel CV biomarkers, inosine/hypoxanthine, in an ongoing study at our institution.
