Background: Thromboelastography evaluates the dynamics of coagulation, using systems such as Thromboelastography (TEG®) and Rotational Thromboelastometry (ROTEM®). While the ROTEM gamma and delta systems require manual pipetting, the ROTEM® Sigma innovates by integrating lyophilized reagents into a cartridge, simplifying the process by directly inserting the cartridge into the analyzer.

Aims: Validate the performance of the Sigma versus Delta platform, identifying advantages and disadvantages.

Methods: Citrated blood samples from 56 adult outpatients, without known coagulopathy, on average 57 years old, were analyzed on both platforms, evaluating correlation and agreement in the parameters.

Results: In the INTEM test, the correlation is modest (0.40) with 83% agreement, indicating that the majority of results are within acceptable limits. EXTEM reveals low correlation in CT, but high agreement (95%). FIBTEM shows agreement in the A10, A20 and MCF parameters (93%, 92% and 88%, respectively) with good interplatform correlation. In APTEM, there is no good correlation in any parameter. In HEPTEM, divergences in all parameters, with CT being the most significant, with low correlation (74%). Discrepancies in reference values generate weak correlation. Despite some out-of-bounds results, most remain acceptable.

Conclusion(s): The ROTEM Sigma platform offers safety by eliminating manual manipulation but faces challenges such as a lack of NATEM reagent and limit the inclusion of further tests. Divergences in results demonstrate the need to interpretate carefully the data. Tests in patients with abnormal results is mandatory to understand the impact in medical decisions.
